Description

IPS Empress® Direct Color is a light-curing characterization material for the

application of individualized characteristics in direct and indirect composite resin

restorations and in CAD/CAM veneers fabricated chairside. Furthermore, it is

suitable for masking discoloured tooth structure. IPS Empress Direct Color is

applied below or between the composite resin layers.

Composition

The monomer matrix (76–89.5 wt%) is composed of bis-GMA, urethane dimeth

-

acrylate and triethylene glycol dimethacrylate. The filler material consists of

highly dispersed, silanized silicon dioxide (9–15.5 wt%). Additional components

include stabilizers, initiators and pigments (< 9 wt%).

Contraindications

The material should not be used

– if the patient is known to be allergic to any of the components of

IPS Empress Direct Color.

– if an adequately dry working field cannot be established or if the stipulated

working technique cannot be applied.

– directly on dentin without dentin protection/dentin bonding agent.

Side effects

In rare cases, components of IPS Empress Direct Color may lead to sensitization.

In such cases, the material should not be used.

Interactions

Phenolic substances, e.g. containing eugenol/clove oil, inhibit the polymerization

of methacrylate-based materials. Therefore, materials containing these

substances must not be used in conjunction with IPS Empress Direct Color.

Discolouration may occur in combination with cationic mouthwashes, plaque

disclosing agents and chlorhexidine.

Application

IPS Empress Direct Color is used together with composite resin filling materials.

In addition, it is used to characterize CAD/CAM veneers fabricated chairside

(IPS Empress CAD), according to a special technique. Conditioning is carried out

according to the instructions for use of the corresponding restorative/luting

composite. IPS Empress Direct Color is dispensed from the syringe and applied

with a brush or a probe. Furthermore, the syringes are supplied with thin,

prebent metal cannulas with which IPS Empress Direct Color can be placed

directly on the restorative material. In order to apply very small portions of the

material, it is advisable to wipe the cannula with a cellulose pad from time to

time to remove any excess material. The shade intensity can be reduced and the

consistency thinned by mixing in Heliobond. Rubber dam isolation

(e.g. OptraDam® Plus) is recommended.

Preparatory steps

1. Remove the syringe cap by twisting it until it snaps and then pulling it off the

syringe.

2. Attach the applicator tip (Fig. 1) and twist it until it is flush with the syringe

(Fig. 2).

A) Characterization of fillings

1. The composite resin (e.g. IPS Empress Direct, Tetric EvoCeram) is applied

according to the directions of the manufacturer.

2. If IPS Empress Direct Color is used on large patches, it can be applied during

the build-up of the restoration and then covered with the restorative material.

In order to simulate enamel cracks and discoloured fissures, a notch is carved

in the appropriate place in the last composite increment (“enamel” layer). The

notch is filled with IPS Empress Direct Color using either the syringe tip or any

other suitable instrument. Composite material is used to cover the notch

starting from the sides. As a result, the characterization material is placed

within the last composite layer and is therefore protected from wear.

Alternatively, IPS Empress Direct Color is applied after the filling has been

built up:

A “characterization pattern” is ground in the filling material. Next, the pattern is

etched with 37% phosphoric acid gel (e.g. Total Etch) for 15 s. The gel is

subsequently rinsed off with water and the pattern is dried with air free of oil and

moisture. In order to establish a sound bond with the polymerized restorative,

the cleaned surfaces should be coated with Heliobond. Subsequently,

IPS Empress Direct Color is applied with a brush or any other suitable instrument

and polymerized according to the exposure time and the light intensity defined

in Table 1.

The maximum layer thickness is 0.5 mm for all shades except Brown. The

maximum thickness for Brown is 0.1 mm. Finally, the characterization material is

covered with composite resin (e.g. IPS Empress Direct, Tetric EvoCeram) and

polymerized according to the directions of the manufacturer.

B) Characterization of labfabricated composite restorations

In the build-up of inlays and onlays, IPS Empress Direct Color is used according to

the procedure described for direct restorations (Section A).

C) Characterization of CAD/CAM restorations fabricated chairside

1. Try-in and subsequently adjust the veneers.

2. The surfaces of the ceramic veneers are cleaned and conditioned according to

the directions of the manufacturer.

3. A thin coating of Heliobond is applied on the conditioned inner surfaces of the

veneers and dispersed to a very thin film with blown air.

4. An extremely small amount of IPS Empress Direct Color is applied in a thin

layer according to the individual requirements. It is important to note that

excessively thick shade layers will compromise the fit and appearance of the

restoration. Adjustments with rotating instruments as well as additional try-ins

after characterization must be avoided, as the bond to the luting material is no

longer ensured.

5. Heliobond and the staining materials are polymerized according to Table 1.

6. The veneers are placed according to the directions of the manufacturer of the

luting composite.

Special instructions

1. Only the cannulas designed for the application of IPS Empress Direct Color

should be used. Other types of cannulas may not fit the syringe properly and

may become loose during the application procedure.

2. IPS Empress Direct Color is best used when it has attained ambient temperature.

At refrigerator temperatures, the contents of the syringe may be difficult to

dispense.

3. IPS Empress Direct Color must not be exposed to bright light during its

application.

4. In order to ensure long-lasting shade effects, IPS Empress Direct Color must

not be applied as the final layer on the surface of the restoration. If possible it

should be placed as an intermediate layer or in carved notches.

5.

If IPS Empress Direct Color is applied directly in the mouth of the

patient, the application cannula of the syringe must only be used for

one patient due to hygienic reasons (prevention of cross- contamination

between patients).

6. Oxidizing disinfectants must not be used to disinfect syringes.

Warning

Contact of uncured IPS Empress Direct Color with skin, mucous membrane and

eyes must be avoided. Unpolymerized IPS Empress Direct Color may have a slight

irritating effect and lead to a sensitization to methacrylates. Commercial medical

gloves do not provide protection against the sensitizing effect of methacrylates.

Storage and hygiene instructions

– Storage temperature: 2–28 °C (36–82 °F).

– IPS Empress Direct Color must not be used after the indicated date of

expiration.

– Shelf life: See date of expiration on the syringe label or packaging.

– Syringes must be sealed immediately after use. Exposure to light may lead to

premature polymerization.

Zusammensetzung

Die Monomermatrix (76–89.5 Gew.%) besteht aus bis-GMA, Urethandi meth acrylat

und Triethylenglyoldimethacrylat. Der Füllstoff besteht aus hochdispersem

Nebenwirkungen

Bestandteile von IPS Empress Direct Color können in seltenen Fällen zu einer Sensi

-

bilisierung führen. In diesen Fällen ist auf die weitere Verwendung zu verzichten.

Wechselwirkungen

Phenolische Substanzen wie z.B. eugenol-/nelkenölhaltige Werkstoffe inhibieren

die Aushärtung von Methacrylat-basierten Materialien. Auf die Verwendung

solcher Materialien zusammen mit IPS Empress Direct Color ist zu verzichten.

In Kontakt mit kationischen Mundwässern sowie bei Plaquerelevatoren und

Chlorhexidin können Verfärbungen auftreten.

Anwendung

IPS Empress Direct Color wird in Verbindung mit Composites sowie für eine

spezielle Technik zur Individualisierung von chairside gefertigten CAD/CAM

Veneers (IPS Empress CAD) eingesetzt. Die Vorbehandlung erfolgt entsprechend

der Gebrauchsinformation des jeweiligen Füllungs-/Befestigungscomposites.

IPS Empress Direct Color kann aus der Spritze entnommen und mit einem Pinsel

oder einer Sonde appliziert werden. Mit den mitgelieferten, vorgebogenen dünnen

Metallkanülen kann IPS Empress Direct Color jedoch auch direkt auf das Restaura

-

tionsmaterial appliziert werden. Um sehr kleine Farbdepots anzulegen, empfiehlt

es sich, zwischendurch Überschüsse von der Kanüle auf ein Zellstoff-Läppchen

abzutupfen. Durch Zumischen von Heliobond lässt sich die Farbintensität ab

-

schwächen und die Konsistenz verdünnen. Eine sichere Trockenlegung, bevor-

zugt mit Kofferdam (z.B. OptraDam® Plus) ist erforderlich.

Vorbereitende Tätigkeiten

1. Entfernen der Spritzen-Kappe durch Drehen über einen spürbaren Widerstand

und anschliessendem Abziehen.

2. Aufsetzen der dafür vorgesehenen Kanüle (Abb. 1) und bündiges Eindrehen

(Abb. 2).

A) Verwendung zur Charakterisierung von Füllungen

1. Verarbeitung des Compositematerials (z.B. IPS Empress Direct, Tetric EvoCeram)

gemäss Herstellerangaben.

2. Bei flächigem Auftrag kann IPS Empress Direct Color direkt während des

Füllungsaufbaus appliziert und überschichtet werden. Zur Simulierung von

Schmelzsprüngen und Fissurenverfärbungen eine Kerbe an der betreffenden

Stelle in die letzte („Schmelz-“) Composite-Schicht modellieren. IPS Empress

Direct Color in diese direkt aus der Kanüle oder mit einem geeigneten Instrument

einlegen. Anschliessend das Composite von den Seiten der Kerbe her wieder

zumodellieren. Das Farbdepot kommt in eine Falte der letzten Composite

-

schicht zu liegen und ist somit vor Verschleiss geschützt.

Alternativ nachträgliches Einlegen von IPS Empress Direct Color:

Einschleifen eines „Charakterisierungsmusters“ in das Füllungsmaterial, 15 s mit

37%igem Phosphorsäuregel (z.B. Total Etch) reinigen, dieses mit Wasser abspülen

und anschliessend mit öl- und wasserfreier Luft trocknen. Um einen guten Ver

-

bund zum polymerisierten Restaurationsmaterial zu gewährleisten, sollte die ge-

reinigte Fläche mit Heliobond benetzt werden. Anschliessend IPS Empress Direct

Color mit einem Pinsel oder einem anderen geeigneten Instrument applizieren

und entsprechend den Angaben zur Belichtungszeit (Exposure Time) und Licht-

intensität (Light Intensity) der Tabelle 1 polymerisieren. Das maximale Inkrement

beträgt für alle Farben ausser Braun 0.5 mm, für Braun maximal 0.1 mm. Zuletzt

mit Composite-Material (z.B. IPS Empress Direct, Tetric EvoCeram) überschichten

und gemäss Herstellerangaben polymerisieren.

B) Verwendung zur Charakterisierung von indirekten Composite-

Restaurationen

Die Anwendung von IPS Empress Direct Color kann analog zum Vorgehen bei

direkten Restaurationen während der Schichtung von Inlays/ Onlays erfolgen

(Abschnitt A).

C) Vorgehen bei Verwendung zur Charakterisierung von chairside

CAD/CAM-Restaurationen

1. Einprobe und abschliessende Anpassung der Veneers.

2. Reinigung und Konditionierung der Innenseite der Keramikveneers gemäss

Herstellerangaben.

3. Auftragen einer dünnen Schicht Heliobond auf die konditionierte Innen fläche

der Veneers und Verblasen dieser Heliobondschicht zu einem ultradünnen

Film mittels Luftstrom.

4. Auftragen feinster Mengen von IPS Empress Direct Color gemäss individuellen

Charakterisierungswünschen in dünner Schicht. Es ist unbedingt zu beachten,

dass zu dicke Farbschichten die Passung sowie das ästhetische Erscheinungs

-

bild beeinträchtigen können. Korrekturen mittels rotierenden Instrumenten

sowie eine erneute Einprobe nach der Charakterisierung sind zu vermeiden,

da hierdurch der Verbund zum Befestigungsmaterial nicht mehr gewährleistet

ist.

5. Polymerisation von Heliobond und Malfarben gem. Tabelle 1.

6. Eingliederung des Veneers gemäss Herstellerangaben des Composite-

befestigungsmaterials.

Besondere Hinweise

1. Es sind ausschliesslich die für IPS Empress Direct Color vorgesehenen Kanülen

zu verwenden. Bei anderen Kanülen besteht das Risiko des plötzlichen Lösens

während der Applikation.

2. IPS Empress Direct Color soll bei Umgebungstemperatur verwendet werden.

Bei Kühlschranktemperatur kann das Auspressen erschwert sein.

3. IPS Empress Direct Color während der Applikation nicht intensiver Beleuchtung

aussetzen.

4. Um einen dauerhaften Farbeffekt zu gewährleisten, soll IPS Empress Direct

Color nicht als letzte Schicht auf die Oberfläche appliziert werden, sondern

nach Möglichkeit als Zwischenschicht oder in modellierte Kerben.

5.

Wird IPS Empress Direct Color direkt im Mund des Patienten appliziert,

so ist die Applikationskanüle der Spritze aus hygienischen Gründen nur

für einen Patienten angezeigt (Vermeidung von Kreuz infek tionen

zwischen Patienten).

6. Keine Desinfektion der Spritze mit oxidierenden Desinfektionsmitteln.

Warnhinweis

Kontakt von unausgehärtetem IPS Empress Direct Color mit Haut/Schleimhaut

und Augen vermeiden. IPS Empress Direct Color kann in unausgehärtetem Zu

-

stand leicht reizend wirken und zu einer Sensibilisierung auf Methacrylate führen.

Handelsübliche medizinische Handschuhe schützen nicht vor Sensi bilisierung auf

Methacrylate.
